Your First Visit

Thank you for your interest in Wise Decision, we are excited to welcome you to our practice and help you through the process.

During your first visit to our practice, you’ll have the chance to get to know Dr. Johnson and our incredible team. You’ll receive a full assessment and discuss options for treatment.

Before your appointment, please take some time to fill out patient registration forms. If you do not have a chance to complete these forms, please arrive at the office early to complete the necessary paperwork.

What to Bring

Before you arrive at your appointment, please make sure to have the following:

• Valid identification
• If applicable, referral slip and X-rays
• A list of current medications with doses
• Medical and dental insurance information
• Form of payment

Please note: If you are under 18 years old, you must be accompanied by a parent or legal guardian to your consultation.

What to Expect

Welcome to Wise Decision! You have truly made the smartest choice by choosing our practice for your oral health needs. Your journey to optimal oral health begins with a consultation appointment. Dr. Johnson will perform an oral examination to fully assess your issue, review your health history, and discuss your hopes for treatment.

After your treatment plan is crafted, you’ll schedule a second appointment for the procedure itself. We will provide any relevant information and detailed instructions for steps you will need to take before your surgery date.

Thanks to our cutting-edge technology, we’re able to crystal clear scans of your child’s mouth to create the most accurate treatment plan.

Panorex Scanners

Panorex scanners enhance our ability to quickly and accurately generate a diagnosis by rotating around the head, capturing a comprehensive view of the mouth and its surrounding areas.
