Complete our secure online referral form below. Please include the patient’s contact information, relevant medical history, and any imaging or records you wish to share. Our team will review the submission and contact the patient to schedule a consultation — typically within one business day. You can also use our digital referral sheet.
Refer by Phone or Fax
If you prefer to refer by phone, call us at (952) 952-1100. You can also fax referral forms and patient records to (952) 952-1101. Be sure to include your practice name and contact information so we can keep you updated on your patient’s progress.

What to Expect After You Submit a Referral
We believe in collaborative care, clear communication, and seamless patient handoffs for every referred practice.
Our Response Timeline
Once we receive a referral, we prioritize timely scheduling. A member of our surgical team will reach out to your patient within 24 business hours to set up a consultation. We understand that your patient’s condition may require urgent attention; for emergency referrals, please indicate that in your submission, and we will accommodate a same-day evaluation whenever possible.
What We’ll Communicate Back to You
We believe in collaborative care. After the consultation, we will send you a summary of findings and the recommended treatment plan. Following the procedure, you will receive a detailed operative report and post-operative instructions. If we identify any additional findings — such as the need for anesthesia support for medically complex patients — we will coordinate with your office before proceeding. Our anesthesia options, including IV sedation with a double board-certified pediatric anesthesiologist on-site, are designed to handle even the most anxious or medically involved patients safely.
