We recommend bringing comfort items, communication devices, or any support tools the patient uses regularly. Arrive with your insurance card, identification, and a list of current medications. If you have questions about what to bring, just call ahead. Getting to us is straightforward: dedicated accessible parking spaces sit alongside access aisles for easy van and vehicle unloading, the main building entrance is step-free with a flat threshold and automated push-button doors, and a fully accessible elevator just inside the main lobby brings you up to our second-floor office. Our floor has fully compliant ADA restrooms with grab bars, turning clearance, and accessible sinks.
Before Your Appointment: Registration Forms
Before your appointment, please take some time to fill out your patient registration forms through our patient portal. If you do not have a chance to complete these forms ahead of time, please arrive at the office early so we can help you complete the necessary paperwork without rushing your visit.

Your journey to optimal oral health begins with a consultation appointment. During your first visit you will have the chance to get to know Dr. Johnson, Dr. Swift, and our team. They will perform an oral examination to fully assess your issue, review your health history, and discuss your hopes for treatment.
After your treatment plan is crafted, you will schedule a second appointment for the procedure itself. We will provide all relevant information and detailed instructions for the steps you need to take before your surgery date.

Options include IV sedation and general anesthesia, both monitored continuously by our anesthesia team.
